{"id":621,"date":"2020-12-01T02:39:19","date_gmt":"2020-12-01T02:39:19","guid":{"rendered":"https:\/\/ftd.cognihealth.in\/?page_id=621"},"modified":"2024-11-01T03:26:28","modified_gmt":"2024-11-01T03:26:28","slug":"why-has-it-happened","status":"publish","type":"page","link":"https:\/\/neurosymptoms.org\/en\/causes\/why-has-it-happened\/","title":{"rendered":"Why has it happened?"},"content":{"rendered":"\n<p>What could be the reasons that lead to the change in nervous system function described on the <span style=\"text-decoration: underline;\"><a href=\"\/causes\/how-has-it-happened\/\" data-type=\"URL\" data-id=\"\/causes\/how-has-it-happened\/\">page describing &#8216;how does FND happen&#8217;<\/a><\/span>?<\/p>\n\n\n\n<p>The answer seems to be that there are many potential reasons why an individual can become vulnerable to FND. These reasons can be divided up in to:<\/p>\n\n\n\n<ul><li>Predisposing Factors:&nbsp;things that happened before the symptoms started<\/li><li>Precipitating or Triggering Factors: things that happened at the time the symptoms started<\/li><li>Perpetuating Factors: things that happened after the symptoms started<\/li><\/ul>\n\n\n\n<p>These factors can be<\/p>\n\n\n\n<ul><li>Biological:&nbsp;for example there probably is a genetic vulnerability to these symptoms;<\/li><li>Psychological:&nbsp;for example when you worry that a headache might be due to a brain tumour it tends to make the headache worse and<\/li><li>Social: for example, if you don&#8217;t enjoy your job and your boss is mean to you, you will tend to feel worse.<\/li><\/ul>\n\n\n\n<p>This is called the &#8220;biopsyschosocial model of illness&#8221;. It doesn&#8217;t just apply to FND. All illness has a psychological and social dimension. Someone with multiple sclerosis who feels miserable or has a lot of debt will tend to experience worse symptoms than someone with multiple sclerosis who is not unhappy and has no other life stresses.<\/p>\n\n\n\n<p>Our models of why people get FND are incomplete. Many of the factors that seem to be relevant are also relevant for lots of other symptoms.<\/p>\n\n\n\n<p>Its still often unclear why someone with these vulnerabilities should for example, become weak down one side, or start to develop a tremor.<\/p>\n\n\n\n<p>I have explained some ideas about why these individual symptoms might happen on the relevant pages (see <a href=\"\/symptoms\/fnd-symptoms\/functional-limb-weakness\/\" data-type=\"URL\" data-id=\"\/symptoms\/fnd-symptoms\/functional-limb-weakness\/\">weakness<\/a>,&nbsp;<a href=\"\/fnd-symptoms\/functional-sensory-symptoms\/\" data-type=\"URL\" data-id=\"\/fnd-symptoms\/functional-sensory-symptoms\/\">sensory symptoms<\/a>, <a href=\"\/symptoms\/fnd-symptoms\/dissociative-non-epileptic-attacks-blackouts\/\" data-type=\"URL\" data-id=\"\/symptoms\/fnd-symptoms\/dissociative-non-epileptic-attacks-blackouts\/\">seizures<\/a>, <a href=\"\/symptoms\/fnd-symptoms\/functional-movement-disorder\/\" data-type=\"URL\" data-id=\"\/symptoms\/fnd-symptoms\/functional-movement-disorder\/\">movement disorder<\/a>).<\/p>\n\n\n\n<p>The picture below shows some factors which can be relevant in the development of functional symptoms. But when you look at this remember that people vary tremendously in how many of these factors are relevant. For some people, many factors are relevant. For others, only one or two are<\/p>\n\n\n\n<p>It is especially important to state that:<\/p>\n\n\n\n<p>YOU DO NOT HAVE TO BE STRESSED. DEPRESSED OR ANXIOUS TO DEVELOP FND<\/p>\n\n\n\n<p>YOU DO NOT HAVE TO HAVE EXPERIENCED AN ADVERSE CHILDHOOD TO DEVELOP FND<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" width=\"873\" height=\"553\" src=\"\/wp-content\/uploads\/2021\/06\/Aetiology-and-MEchanism.jpg\" alt=\"\" class=\"wp-image-2682\" srcset=\"https:\/\/neurosymptoms.org\/wp-content\/uploads\/2021\/06\/Aetiology-and-MEchanism.jpg 873w, https:\/\/neurosymptoms.org\/wp-content\/uploads\/2021\/06\/Aetiology-and-MEchanism-300x190.jpg 300w, https:\/\/neurosymptoms.org\/wp-content\/uploads\/2021\/06\/Aetiology-and-MEchanism-768x486.jpg 768w\" sizes=\"(max-width: 873px) 100vw, 873px\" \/><figcaption> A model of how various risk factors lead to and perpetuate FND &#8211; a disorder going on in the brain<\/figcaption><\/figure>\n\n\n\n<p>Lets look at some of these things in more detail<\/p>\n\n\n\n<h3>Predisposing factors<\/h3>\n\n\n\n<p>We know from studies that people with FND are about 2-4 times more likely to have had adverse experiences in childhood or adult life like physical or sexual abuse. That still means most have not had these things. But how could that make someone more vulnerable to FND? It probably happens for a number of reasons<\/p>\n\n\n\n<ul><li>Having bad experiences sensitises your &#8216;fight or flight&#8217; response so it &#8216;alarms&#8217; more easily later in life, even when there is only a minor injury or illness.<\/li><li>Injury and abuse promotes a tendency to &#8216;dissociate&#8217; as a response to horrible, threatening and painful experiences. That &#8216;pattern&#8217; can get re-activated later in life when there is another threatening experience<\/li><\/ul>\n\n\n\n<p>If you are someone who has had an adverse experience in the past, then you may get a hopeless feeling that the &#8216;damage has already been done&#8217;. This is not the case. Treatment can help look at these kinds of factors if they are present and help you to &#8216;retrain&#8217; longstanding patterns of response. This can help you come to terms with whats happened but also manage &#8216;threats&#8217; to your body more easily.<\/p>\n\n\n\n<p>But other predisposing factors are just as important. Other common physical illnesses like migraine or joint hypermobility syndrome\/Ehlers Danlos type 3 seem to make people more likely to get FND. In fact other medical conditions are one of the commonest risk factors for FND generally.<\/p>\n\n\n\n<p>There is some evidence that people with FND are more likely to be obsessional, which makes them good workers, with a good attention to detail, but also not good of &#8216;letting go&#8217; when an abnormal program gets lodged in their brains as part of FND. This can also link to a tendency  to be &#8216;always on the go&#8217;. People who are &#8216;always on the go&#8217; often are not good at being unwell when they have a minor illness or may not have recognised they were running themselves down. This is sometimes one reason why they collapse.<\/p>\n\n\n\n<h3>Precipitating or Triggering Factors<\/h3>\n\n\n\n<p>Throughout this site Ive discussed the kinds of triggering factors that can be important in FND. For example<\/p>\n\n\n\n<ul><li>Limb injury triggering FND limb weakness<\/li><li>Faints or panic attacks triggering Functional Seizures<\/li><li>Drug Side effects triggering functional tremor<\/li><li>Prolonged fatigue<\/li><li>Migraine<\/li><li>Infectious illness<\/li><li>Extreme stress <\/li><\/ul>\n\n\n\n<h3>Perpetuating Factors<\/h3>\n\n\n\n<p>These are all the things that once you become ill just keep making it worse for example<\/p>\n\n\n\n<ul><li>Not getting a diagnosis<\/li><li>Not being believed by health professionals or friend and family<\/li><li>Going on to strong medication like opiates that tend to make FND worse<\/li><li>Getting depressed or anxious about the situation you find yourself in<\/li><\/ul>\n\n\n\n<p>You may want to look at this table and decide what your own personal risk factors were and whether <a href=\"\/treatment-2\/\" data-type=\"URL\" data-id=\"\/treatment-2\/\">anything can be done about them<\/a>.
